    How can you receive better health care? With a Patient-Centered Medical Home, you receive an innovative, team-based approach to your care, which makes it accessible, comprehensive and coordinated.

    Wellmont Medical Associates has achieved Level 3 Patient-Centered Medical Home status from the National Committee for Quality Assurance. Level 3 is the highest achievable recognition for a medical group.

    What does this mean for you?

    A Patient-Centered Medical Home is a medical office or clinic focused on you and the partnership you have with your doctor. And a team of health professionals work together using modern technology and superior knowledge to provide you with the best possible care as we help you reach your health goals.

    Who is part of your medical home care team?

    Along with your primary doctor, the team may include a nurse and a health educator, as well as other health care professionals, such as a pharmacist or physical therapist. These team members are health coaches who help you get healthy, stay healthy and obtain the care and services that are right for you.

    Family members who care for you are also part of the team, and you, of course, are the most important member and have the ultimate say in the direction your care is going.

    How does a medical home work?

    In a medical home, your care team:

    • Is available for you
      You can communicate with your care team by phone or electronically using the MyWellmont patient portal, and you can get an appointment quickly.
    • Knows you and your health history
      The team knows your personal or family situation to create treatment options that will work for you.
    • Makes sure you understand your condition
      Your care team doesn't want you to leave with any unanswered questions and will strive to make sure you understand all your health care options.
    • Helps you coordinate your health care
      If you need a specialist for a specific condition, your team will help you find the right one for your needs. Your tests and treatments by other doctors will be sent to your primary care doctor so you have a centralized place for your medical history.
